Documents and fee required when forming a community interest public limited company
You are required to deliver to Companies House the following completed documents, with a fee:

- Form 12
- Form CIC 36 (Application to form a Community Interest Company, inclusive of declarations)
- Models of Memoranda and Articles of Association - Templates. If you are not using one of the Model Memoranda and Articles of Association please click on the following: Important information
- Cheque for £35 payable to Companies House
- CIC36 Application Form - Continuation Sheets (use only if needed)
Please note a public community interest company may not commence business or exercise any of its borrowing powers unless the Registrar of Companies has issued a certificate under section 117 of the Companies Act 1985 (commonly known as a trading certificate). To obtain this certificate a PLC CIC is required to complete the following application form:
